What is a torn cruciate ligament?

The anterior and posterior cruciate ligaments stabilise the knee joint and ensure controlled movement. If one of these ligaments tears – usually the anterior cruciate ligament – the knee loses stability. Typical signs are a sudden popping sound, pain, swelling and a feeling that the knee is giving way.

Diagnosis

The diagnosis is made through a combination of:

  • Clinical examination (stability tests)
  • Imaging (MRI as the gold standard)
  • Medical history (how the injury occurred, symptoms)

Consequences of a torn cruciate ligament

If left untreated, a torn cruciate ligament can lead to chronic instability, meniscus and cartilage damage and early osteoarthritis. The risk of secondary damage is particularly high in physically active people.

When is cruciate ligament surgery necessary?

Cruciate ligament surgery is particularly advisable when:

  • there is an unstable knee joint,

  • sports involving twisting and jumping (e.g. football, skiing) are to be resumed,

  • accompanying injuries (meniscus, cartilage) are present,

  • despite physiotherapy sufficient stability is not achieved.

In certain cases – for example with low everyday strain – a torn cruciate ligament can also be treated without surgery. We will advise you individually.

Cruciate ligament surgery: surgical procedures

Depending on the findings and lifestyle, different procedures are used:

  • Cruciate ligament reconstruction with the body’s own tendons (e.g. semitendinosus or patellar tendon)

  • Minimally invasive arthroscopic technique (keyhole surgery)

  • Revision operations in the case of a repeat tear

All procedures aim to restore the natural stability of the knee.

Cruciate ligament surgery: the procedure

1

Preparation and examinations

Before the operation there is detailed planning, blood tests, imaging and a comprehensive consultation. Important: What to do until the operation? Gentle movement, reducing inflammation and, where appropriate, preparatory physiotherapy.

2

Procedure and duration in the operating theatre

The cruciate ligament operation takes usually 60–90 minutes and is generally carried out under general or partial anaesthesia.

3

After the operation

Pain management, mobilisation and the first exercises begin immediately after the operation. A short inpatient stay is often advisable.

4

Rehabilitation phase

Rehabilitation is crucial to success. It takes place in stages over several months and includes physiotherapy, muscle building and coordination training.
